What You'll Do:
As a Clinical Evaluator, you will be responsible for conducting thorough assessments of residents' health conditions, functional abilities, and care requirements.
Your evaluations will drive the development of personalized care plans, ensuring the highest standard of care and well-being of our residents.

Responsibilities:
Perform comprehensive assessments of physical, mental, and emotional health to determine appropriate care settingEvaluate functional abilities and care needs through detailed observations and assessmentsCollaborate with interdisciplinary teams to develop individualized care plansDevelop and maintain positive relationships with area hospitals and community agenciesEstablish and maintain relationships with social workers, case managers and other contacts in the healthcare and payor communityEducate residents and families about facility services, procedures, and care expectationsMonitor and reassess residents' progress and update care plans accordinglyAdvocate for residents' well-being and coordinate with healthcare providers to address needsDocument assessments and care plans accurately and timelyIf you are a skilled and empathetic healthcare professional with a passion for assessing and advocating for patient care needs, join our team as a Clinical Evaluator.

What You'll Bring:
Qualifications of a Clinical Evaluator include:
Nursing or Social Work degree, with valid state license or certification preferred3 years of clinical or case management experience in long-term careExperience in discharge planning and marketing activityCompassionate and empathetic approach to patient care Interest in the nursing needs of the aged and the chronically ill with the ability to work with bothStrong clinical skills and critical thinking abilities Excellent communication and interpersonal skillsAbility to work effectively in a multidisciplinary team environment-:
